League Forms
Game Roster
Game Roster must be completed by each team and given to the referee with player cards prior to each game.
Team Agreement Form
This form should be completed each season when teams submit their deposit to the league.
Player Registration Form
Every player that participates in this league must of a player registration form on file as well as a photo emailed to the
registrar. Each fall season all players must complete a new registration form, although if photo is on file then a new
photo does not need to be submitted.
USSSA Team Roster
Each season the team manager must submit a new USSSA team roster which lists all the players playing on their team
for the given season.
