Registration Form - Please fill out completely and be sure to sign in the player signature box at the bottom of the page. Send it to the registrar by the Tuesday before the 1st game you are to play. |

Passport size digital photo - New players must provide a color passport size digital photo. Face shot must be the size of a passport photo. Small face shots will NOT be accepted.

Drivers license copy - a xerox copy of your driver's license or other acceptable proof of age must be part of your new player package.
NOTE: You may white out/black out the address on the license for security reasons. |
