REGISTRATION INFORMATION

         
 
  1. Submit a minimum roster of 13 player’s maximum of 22, registration fees, and complete registration forms for each player.


  2. Each player must have: PWSL waiver form, copy of driver’s license, and a check or money order (PWSL waiver is available online at www.pwsl.org).


  3. Players can black out the street address and driver’s license number on the id form security reasons. The picture, name, and date of birth MUST be clear.


  4. Submit a digital color picture for each player (easiest via CD) with each player listed with Last name, First name, Team Name (photos must be color head shots, no sunglasses, hats, or other items that must be cropped out)


  5. Write the name of your team on all checks.  Checks should be made payable to PWSL and mailed to P.O. Box 420508, San Diego, CA 92142.  For the initial team registration, checks can be submitted with the team packet.  Do not submit cash for registration fees.  Checks and money orders are accepted.


  6. Do not have your players call the Registrar for information on how to register; this is the Team Rep’s Responsibility.


  7. Once the season starts, please submit all registration/waiver forms by Tuesday prior to the game in which you want the player to play.  There will be NO exceptions to this rule.


  8. If your team needs players, create a team want ad using the site bulletin board under the “Team Representative” drop down menu found on the http://www.pwsl.org website.


  9. If you need to drop a player or if the player is changing teams the player must fill out a PWSL Resignation / Transfer form and return the player card to the Registrar along with a check for $10 for a new card. Player will not be dropped or transferred without return of the player card.  Each team rep (old team and new team) will then need to submit a new roster. Player cards should only be returned to the Registrar.


  10. Replacement cards will be charged a $10 fee.


  11. If you need to pickup cards from the Registrar please call in advance to make the appropriate arrangements.


  12. Do not call Carole, please refer all questions to the appropriate board member.
